In-home medication management

In- home medication management one of our aim is to provide medication management services and home healthcare units to users and/or their caregivers who are at home with information, specialized health care and support in relation to medication that has been prescribed by a health care professional to home care patients. In order to provide appropriate medication management support and help for our loved ones at home, pharmacists participating in the services will conduct a Medicines Use Review from which they will develop a medication management plan for our loved one that will help detail the specific support needed to meet the needs of the health care units. Whether the user at home that we are providing services is suffering from cognitive decline or a degenerative disease, the services goal is to help them to remain in their home site with home healthcare healthcare as long as possible.

In in-home medication management, the provision of appropriate compliance aids by participating pharmacists will be the foundation of the service. This may include the provision of Monitored Dosage Systems in conjunction with the use of Medication Administration Records for recording purposes.

 

Referrals are made by community / hospital teams when the medication management services user is identified as being unable to self-medicate at home and requires assistance. Maintain a good medication management scheme to improve your health. Before taking any medications, check the label and expiration date. Inform the visiting nurse or therapist if the patient starts or stops taking any medications.

  • Keep all medicine bottle caps in a safe place away from children and pets.
  • Do not combine medications in one bottle.
  • Keep medications, including pills and IV bags, out of direct sunlight.
  • Keep medicines at the proper temperature and notify the visiting nurse, doctor, or pharmacist if you notice any changes in the color, size, or smell of the medicine.
  • Medicines that must be refrigerated:
    • Keep medicines in the refrigerator at the proper temperature.
    • They should not be frozen.
    • Keep cold foods and medications separate.
    • If you have several medications in the refrigerator, keep them separate from one another to avoid any effects.
  • Check that the light in the refrigerator is turned on.
  • Maintain an up-to-date list of the medications the patient is currently taking i.e. proper medication management.
