We collaborate closely with your primary care physician, specialists, and all other members of your health care team, including those in the community such as Adult Day Programs, Elder Services, and Advocacy organizations. To ensure the best possible outcomes, we communicate constantly with everyone involved in your care. Our Nurses are highly skilled and use a patient-family centered approach to help you achieve your goals. This means that the patient and his or her family are involved in every decision made during your care.
