What You Will Do:

As an Essential Home Health Care Wound Care nurse, you will have the unique opportunity to provide one-on-one, skilled nursing care and treatment to patients in their homes who have ostomies, draining wounds, fistulas, pressure sores, and incontinence. Your primary responsibility will be to collaborate with health professionals, including physicians, to develop a home care plan to assist an individual, other clinical professionals, and their caregivers in managing and improving the patient’s condition at home.

Required Qualification

A current and unencumbered license to practice as a registered nurse in the state in which the employee is assigned to work. Board certified with the Wound, Continence, and Ostomy Nurses Society or board eligible with one (1) year to complete boards.

 

A bachelor’s degree in nursing or a higher degree in a related field is required.

A registered nurse with at least two (2) years of clinical care experience is required.

 

CPR certification that is currently valid

 

Current driver’s license, agency-mandated automobile liability insurance, and personal, dependable transportation to conduct home visits are all required.

Preferred Qualification

More than one (1) year of home health experience

 

Experience as a registered nurse with the adult population in a medical – surgical environment

What you will be doing

Performs the wound therapy evaluation visit, joint visits with the skilled nurse, and re-evaluates the patient’s nursing needs at each subsequent visit; initiates, develops, implements, and revises the patient’s Plan of Care in response to identified patient care issues.

Assist physicians in the care of patients who have stomas, draining wounds, fistulas, pressure sores, or incontinence, including stoma sitting, teaching, intermittent self-catheterization, and consultation.

Provides services that necessitate substantial and specialized nursing skills, such as evaluation, selection, and recommendation of supplies and equipment for patients with ostomies, draining wounds, fistulas, pressure sores, incontinence, and any other wound type of injury in accordance with the physician-signed Plan of Care.

Coordinates the plan of Care and ensures patient care continuity by liaising with other appropriate staff.

When necessary, actively participates in Team/Case Conferences and initiates Team/Case Conferences for complex and/or multi-disciplinary patients.

Supervises and instructs home health aides and LPN/LVNs. Changes in the Plan of Care are communicated to the appropriate staff.
