Patients who use our remote monitoring program spend less time in the hospital. With trained virtual health nurses monitoring your signs and symptoms, you can relax at home knowing your care is in good hands.

The Advantages of Remote Monitoring

When your doctor or home health nurse suggests adding remote monitoring to your regular visits, our team gains valuable insight into your needs. The following are some of the advantages of using this service: 

 

  • Specialty cardiac care programs: We offer specialty programs based on remote monitoring for people managing a chronic heart condition at home or recovering from heart surgery. Among these programs are:
    • Diuretic Heart Failure Program: We closely monitor your symptoms if you are at high risk of heart failure. If your doctor decides you need diuretic medication, we can administer it to you at home.
    • Rapid Recovery Program: Following open heart surgery, our virtual health team collects data between nurse visits and collaborates with your doctor to adjust your medications as needed. We will get you to outpatient cardiac rehabilitation as soon as you are able.
  • Health education and coaching: Monitoring vital signs is only one aspect of virtual health. Our nurses will teach you how to interpret your results (such as weight and blood pressure). They will also advise you on when to contact a doctor, your home care nurse, or go to the emergency room.

What to Expect from Remote Monitoring

Your nurse will determine whether remote monitoring is a good fit for you during your initial home care visit. If you choose to participate, your monitoring equipment will be delivered following your initial assessment.

The activation of the equipment alerts our virtual health team. A nurse will contact you right away to provide instructions and answer any questions you may have. Every day, you will gather information about your health in two ways:

Check your vital signs. You’ll use simple equipment to collect vital signs such as: in less than 3 minutes per day.

 

    • Heart Rate
    • Blood pressure
    • Oxygen level
    • Body weight
    • Temperature

 

Examine your symptoms. Each morning, your tablet illuminates, prompting you to answer questions about your symptoms.

A virtual health nurse contacts you and arranges any additional care you may require if your vital signs or symptoms are concerning.

Your Remote Monitoring Instruments

Your remote monitoring equipment is simple to use and ready to use right away. The provided electronic tablet registers your vital signs as they are taken via a wireless connection. The information is accessed by virtual health nurses via a secure connection to the tablet. Your remote monitoring equipment may include the following items, depending on your requirements:

 

  • Tablet computer: You use a small tablet to assess your symptoms on a daily basis. Simply connect it to a power source and turn it on.
  • Blood pressure cuff: The cuff is battery-powered and automatic. Simply place it on your upper arm and press the start button.
  • Pulse Ox: This device measures your oxygen level as well as your heart rate.
  • The digital scale has a large display and reads the result aloud.
