Licensed Practical Nurse | Full Time
What You Will Do:
As a Licensed Practical Nurse with Essential Home Health Care, you will provide one-on-one skilled nursing care and treatment to patients in their homes by utilizing a comprehensive and patient-specific care plan, innovative solutions, and a preferred technology for point-of-care documentation. You will be able to spend quality time with the patient in their home, promoting their health and independence. You will have autonomy as well as the support and collaboration of a large team of healthcare professionals.
Required Qualification
A valid and unencumbered license to practice as a practical/vocational nurse in the state in which the employee is assigned to work.
One (1) year of clinical experience as a Licensed Practical Nurse/Licensed Vocational Nurse is required.
CPR certification is current.
To complete home visits, you must have a valid driver’s license, reliable transportation, and the agency’s liability insurance.
Preferred Qualification
Acute care experience as a licensed nurse with adults in a medical – surgical setting.
Previous experience in home health or hospice.
What you will be doing
Implement and adhere to a patient-specific care and treatment plan in collaboration with physicians, patients and their families, nurses, therapists, social workers, and other members of the patient care team.
Use a variety of solutions to educate and promote the patient’s health and independence.
Assess, monitor, document, and report progress of a patient’s health and condition using required documents such as clinical progress notes and within established timelines, using a clinician-preferred technology.
Participates in Team/Case Meetings.
Other responsibilities may be assigned.
