Even if they have a chronic disease like heart failure, most people prefer to live at home. Our home health care team will collaborate with your doctor to help you manage heart failure symptoms at home, allowing you to stay at home and reduce your chances of returning to the hospital.
COPD Program
When living with a chronic disease like COPD, most people prefer to receive care at home (chronic obstructive pulmonary disease). Our home health care team will collaborate with your doctor to develop a COPD management plan that will allow you to remain at home and live as independently as possible.
Fall Reduction Program
Falls are common in older adults, but they are not a normal part of the aging process. Our Fall Reduction Program can assist you in lowering your risk of falling in the comfort of your own home.
Diabetes Care Program
Our home health diabetes program enables qualified patients to manage their symptoms and reduce their chances of returning to the hospital. We collaborate with your healthcare team to provide skilled nursing, therapy, home health aides, and other diabetes-management services in the comfort of your own home.
Wound Care Program
Many of our patients require specialized care, which Essential Home Health Care recognizes. That is why we have a group of nurses who are specifically trained to treat complex wounds. To help patients achieve the best possible treatment outcomes, our wound care nurses use evidence-based treatment protocols from the Wound, Ostomy, and Continence Nurses Society and the National Alliance of Wound Care.
Stroke Recovery
Stroke patients require specialized care and rehabilitation. We hire home health aides and live-in caregivers who have received extensive training in stroke recovery and rehabilitation at Essential Home Health Care.
Hip and Knee Replacement Recovery at Home
