Our professional representatives always respond to your concerns in a timely and courteous manner. Indeed, registered nurses are available 24 hours a day, 365 days a year to provide guidance and direction to our aides. Our home health aides and other staff are carefully screened and chosen for their skill, knowledge, dedication, compassion, demeanor, and professionalism.
Flexibility
Often, the need for home care arises unexpectedly, such as when a patient sustains a sudden injury or is discharged from a hospital or other care facility. Other times, a patient only requires assistance for brief periods of time or for specific tasks. As our aides are available for last-minute requests, brief shifts, specialized assistance, and other individualized requests, we gladly tailor our services to your specialized needs.

Medicare Eligibility for Home Health
- A physician must order home health care services.
- The service must be reasonable and required in order to treat the illness or injury.
- A qualifying skilled intermittent service, such as nursing care, physical therapy, occupational therapy, or speech therapy, must be required by the patient.
- Medical social services and home health aide services are only covered if the patient is receiving skilled nursing care, physical or speech therapy, or both.
- The patient must be confined to his or her home*.
- The patient is a Medicare beneficiary.
When all of the conditions and criteria listed above are met, Medicare will pay for an unlimited number of visits.
Definition of Homebound
- Inability to leave the house without making a significant and taxing effort
- A physician certifies the patient’s homebound status.
- Homebound patients are permitted to leave their homes for brief and infrequent absences for non-medical reasons.
- Homebound refers to a patient with a psychiatric condition who is deemed too dangerous to leave their home unattended.
- Consider the need for supportive devices in conjunction with physical limitations.
When the following conditions are not met, the homebound status is not met:
- The patient is capable of driving a car safely.
- Absences from home for social reasons on a regular basis

Receiving Request
It starts with a referral for services from you, your family member/friend or your doctor.
Order From Your Physician
In order to evaluate the appropriateness of your home health, we must request an order from your doctor or one of our consulting physicians to assess your condition.
Evaluation of Patient Profile
Upon receipt of a doctor’s order, a registered nurse is appointed as a case manager. The RN starts the recovery process by first completing a written evaluation of your physical, emotional and social needs. The assessment shall include:
– The safety check of your house.
– A full physical assessment of all systems of the body.
– A comprehensive review of all prescription & OTC drugs at home.
– Verification of medicines, procedures, and medical orders with the prescribing physician.
Get a Nurse/Therapist assigned to you
Appropriate clinicians will be appointed and your treatment will begin. The Licensed Vocational Nurse (LVN) would be your most regular visiting nurse with 1-2 visits per week. Additional services are defined by your Licensed Nurse Case Manager and may include: Certified Home Health Assistance (CHHA), Physical Therapist, Occupational Therapist Speech Therapist, Medical Social Worker or Psychologist.
